Understanding Ureteropelvic Junction Obstruction

Ureteropelvic junction (UPJ) obstruction is a blockage at the point where part of the kidney attaches to one of the tubes to the bladder (ureters). The blockage slows or stops the flow of urine out of the kidney. Urine can then build up and damage the kidney. The obstruction can either be congenital (present since birth), caused by trauma, previous surgeries and kidney stone.

Medical Management

UPJ obstruction in infants does not require treatment since they can outgrow the condition within 18 months of life. However, antibiotics may be prescribed to prevent infection. Persistent obstruction with poor urine outflow and increased risk of kidney damage require surgical intervention. Medical treatment is not recommended in adults, and surgical intervention is necessary to treat the obstruction.

The symptoms of UPJ depend on the age of the patient and differ in adults and infants.

In infants -

Most often, the obstruction in infants can be identified through routine pregnancy scans that present with hydronephrosis (abnormal swelling of the affected kidney). Other potential symptoms include:

  • Blood in the urine (hematuria)
  • Failure to thrive (poor growth)
  • Urinary tract infection accompanied by fever, foul-smelling urine, poor feeding, vomiting, and discomfort while urinating.

In older children & adults -

  • Pain in the side of the abdomen, that intensifies on consuming liquids
  • Presence of abdominal lump
  • Repeated urinary tract infections
  • Blood in the urine (hematuria)
  • Nausea and vomiting
  • Swelling (edema) of the face, especially around the eyes
  • UPJ obstruction mostly occurs in children. It often happens when a baby is still growing in the womb. This is called a congenital condition (present from birth). The blockage is caused when there is -

    • A narrowing of area between the ureter and the part of the kidney called the renal pelvis
    • An abnormal blood vessel crossing over the ureter

    As a result, urine builds up and damages the kidney. Other causes can be due to scar tissue, infection, earlier treatments for a blockage, or kidney stones. UPJ obstruction is the most common cause of urinary blockages in children. It is now commonly found before birth with ultrasound tests. 

Medical researchers haven’t found a link between diet and nutrition during pregnancy and the formation of UPJ obstruction in a developing fetus. You can’t prevent UPJ obstructions that are present at birth.

Adults without a congenital history of UPJ obstruction can reduce the chances of the same - by following a lifestyle that keeps the kidneys healthy and reduces possibility of kidney stones, UTIs and trauma to the kidneys.
